Add Person Information
The Person Table is a Child Table of the central Incident Table. Since it is a
separate table it allows for multiple people to be connected to a single
incident. For example, a van rollover (single incident) sould result in multiple
people being injured.
For each person involved in the incident, you would enter complete information before adding
an additional person.
This is a sample form. No data is actually submitted.
Person Information
Enter information about Person(s) involved in the incident. If there were injuries
or illnesses, you will be prompted to enter information about those after completing
this portion of the form.
Instructions
- Press Check Submission: Check your submission before entering. If you are missing required information, you will be prompted to enter data.
- Press Add Person: If no error messages pop up, your data will be entered and you will be taken to a confirmation page showing your new submission. Only press the Add Person button once.
