Add Illness Information

The Person Table is a Child Table of the central Incident Table. The Person Illness Table (illustrated below) is a Child Table of the Person Table. It allows you to record multiple illnesses for a each person and categorize the type of illness, anatomical location and the severity of that illness.

For example, a person could have a minor eye infection, and a major diabetic incident.

The Sample Form does not allow you to add data.
